RED-S Assessment Tool

Is low energy availability
affecting your health?

This tool is built on the LEAF-Q — the most widely validated questionnaire for detecting low energy availability (LEA) in female athletes. LEA is the root driver of RED-S. The assessment covers the three clinical domains with the strongest evidence: gastrointestinal health, injury history, and menstrual function.

Your period alone
isn't the whole picture

RED-S affects far more than menstrual function — and it doesn't always announce itself with a missing period. You can have seemingly normal cycles and still experience significant hormonal disruption.

This questionnaire is based on the LEAF-Q (Low Energy Availability in Females Questionnaire), the most widely validated tool for screening LEA in female athletes. It assesses three domains — gastrointestinal symptoms, injury history, and menstrual function — which are the strongest clinical markers that energy availability is chronically low.

These three domains don't cover every way RED-S can present, but they represent the root mechanism. A high score here is a meaningful signal. A low score is reassuring — though not an all-clear if you still have symptoms worth investigating.

Signs that RED-S may be
relevant to you

Irregular or absent periods
Frequent illness or injuries
Persistent fatigue despite rest
GI symptoms (bloating, IBS-like)
Stress fractures or bone issues
Cold sensitivity or poor recovery
Controlling food or body weight
High training load + low intake

No diagnosis. No judgment. Just clarity.

Energy Availability Assessment

LEAF-Q Questionnaire

Please answer each question as honestly as you can. There are no right or wrong answers.

Gastrointestinal Symptoms

In the past 3 months, how often have you experienced each of the following?

NeverRarelySometimesOftenVery often

1. Bloating

2. Diarrhoea

3. Constipation

4. Stomach or abdominal pain / cramps

5. Nausea or vomiting

6. Heartburn or acid reflux

7. Excessive gas / flatulence

8. Difficulty or discomfort when swallowing

Step 1 of 3

Please answer all questions before continuing.

Injury History

Low energy availability can affect bone density and recovery, making injuries more frequent and harder to heal.

9. Have you ever experienced a bone stress injury — a stress reaction or stress fracture diagnosed by a clinician or on imaging?

10. In the past 12 months, have you had any injury that required you to stop or significantly reduce your training for more than 2 weeks?

11. Compared to others who train at a similar level, do you feel you get injured more frequently?

Step 2 of 3

Please answer all questions before continuing.

Menstrual Function

Menstrual function is a sensitive marker of hormonal health. If you use hormonal contraception, we'll ask about your cycle history before or between use.

12. Are you currently using any form of hormonal contraception? (e.g. combined pill, mini-pill, patch, injection, implant, hormonal IUD/Mirena)

Since hormonal contraception can suppress or regulate your cycle artificially, the questions below refer to your cycle before starting contraception, or during any extended breaks from it.

13. In the past 12 months, how many menstrual periods have you had?

14. How regular are / were your periods? (Regular = cycle length varies by 7 days or less)

15. How long does / did your menstrual bleeding typically last?

16. How old were you when you had your first period?

Step 3 of 3
